Justifications

Written justification, in the proper format, must be attached to the Purchase Order under the following conditions (click on the form to retrieve it):

• When requesting release from a state contract or a mandatory source:
  
       Attach release form obtained from Materiel Management.

• When requesting a Sole Source for goods or services:
  
       Sole Source Instructions
       Sole Source Document
       Sole Source Non-Technology
       Sole Source Technology
  
       Justification must be signed by the agency head or his/her
       designee and attached to the Purchase Order.

• When requesting the purchase of Used Equipment:
  
       Used Equipment Form

• When requesting the purchase of a Proprietary item:
  
       The determination for the use of proprietary purchasing
       procedures shall be made in advance, in writing, and be
       included in the procurement file.
