In preparing your jury forms at the end of the semester, please follow these steps carefully:

1. Copy the form (below the red line) onto your word processing software.

2. Adjust the form so that the words "beginning of p. 2" actually appear at the top of page two IN YOUR SOFTWARE.

3. Fill out the forms, and be sure that "beginning of p. 2" is still in the appropriate place, and that everything fits appropriately on two pages. Make it look nice. It will go in your permanent file.

4. Print the forms ON TWO SIDES OF THE SAME SHEET OF PAPER. Do not submit two sheets staples together.

5. Submit 3 copies of this form to your applied teacher by the following deadlines: Vocal -1 full week prior to the jury; Instrumental - at the beginning of the jury.  Students performing for the Sophomore Promotional Jury (MUSC282) should submit 7 copies instead of 3.

It is important to follow these instructions.
