The Clinical Experience Placements Handbook (pdf) (Moodle Tutorial pdf) provides an overview of the varying roles of the cooperating teacher, host school principal, university supervisor, and teacher candidate. It also includes suggestions and checklists that can assist you as you actively participate in this phase of the teacher preparation program. One of your responsibilities as a cooperating teacher is to complete the two online documents identified below. These online forms help the OPS and the various program areas evaluate the clinical experience placement and make the improvements necessary to better prepare teacher candidates to meet the placement needs of our partnering PK-12 school divisions.
1. Final Clinical Experience Evaluation (pdf)
a. This assessment should be completed at the end of the 7 1/2 weeks placement.
b. For guidance on how to rate the teacher candidate for each indicator, please refer to the Conceptual Framework Rubrics.
c. The confidential access number for this evaluation was provided in an email from the OPS.
2. Cooperating Teacher Feedback Form (pdf)
a. This assessment should be completed at the end of the placement.
b. Please complete all blanks/fields before submitting.
