All appeals must be received electronically (via this designated webform), no later than 5:00pm on Wednesday June 18, 2008.

NO APPEALS WILL BE ACCEPTED AFTER THIS DEADLINE.

Final appeal decisions will be communicated via provided e-mail, no later than Wednesday, June 25, 2008. In order to avoid a student account hold flag, payment must be received by the Office of Cashiering, no later than close of business on Monday, June 30, 2008.

Payments must be made by personal check...sorry, no credit cards.  To ensure payment accuracy, use the provided pre-addressed Office of Cashiering envelope.  Also include your LU ID # and the designation of "Damage Payment" in your check's memo section.
