Enrollment Verification

To receive a copy of an Enrollment Verification, either stop by the Registration office or print a copy of the form by clicking on Enrollment Verification form . You must include the following information:
                     your name
                     id number
                     the address(es) where the Enrollment Verification should be sent
                     dates to be verified
                     your signature

We will certify your enrollment only for semester(s) for which you are actually registered, not for future semesters.

If you wish to pick up your verification instead of having us mail it, please indicate on the form, otherwise mail completed form to the address listed below. Allow three working days to process your request.
