Committe Action Form (Required - Should be Front Page of Submission Packet)

Proposal Submissions Form (Required)

Description of Research Template (Some version of a description required)

Informed Consent Template (Some version of informed consent required)

Thesis Committee Approval Form (Required if a Master's thesis)

Prince Edward County School Division Approval Form  (Required if collecting data in or from the school systems.  Must be completed and submitted to Superintendent 3 weeks prior to data collection in school systems)
